- Vitals: 92/41; 56RRR; 36.8C
- Intubated at OSH
- GCS: 3T ⁣

He sustained a Right Le Fort III fracture, Left Le Fort II fracture, Bilateral NOE fractures, Bilateral nasal fractures and septal fracture, Right alveolar process fracture extending through hard palate, Right displaced mandibular body fracture and a Left displaced mandibular ascending ramus fracture. ⁣
⁣
He was treated with ORIF of the R Le Fort III fracture, L Le Fort II fracture, R alveolar process fracture, R mandibular body fracture, L mandibular ascending ramus fracture. And closed reduction of the  of b/l nasal fracture, b/l NOE fracture, septal fracture. He was placed in MMF. He went to the SICU after the OR.

Incredible timing, given that physicians have been prominent figures in the news lately due to #covid19 🦠 Frontline providers in certain parts of the US have continued to show up for their patients in spite of abysmal shortages of personal protective equipment (PPE). I may not be on the front lines just yet, but part of training at a large medical center means being ready to jump back in to keep the ship afloat. I’m sure many of us are experiencing a new and foreign sense of vulnerability and our fair share of anxiety about what the landscape will be tomorrow, next week, one month from now. If and when we get sick - Will we bring it home to our children, spouses, and parents? Will it affect our unborn children? Will we have a mild course and recover at home, or a more complicated course requiring hospitalization? Every time a doctor, nurse, PA, NP, tech, environmental services staff member, etc sets foot in the hospital, they are knowingly risking their own health in the name of fulfilling their calling, and that IS admirable. ⠀⠀⠀⠀⠀⠀⠀⠀⠀
If you have friends or loved ones in the medical field, please call/text them. Let them know that you are thinking about them, but also that you are doing your part to fight this battle by:

4️⃣ Supporting apps & businesses that are pitching in to make our “new normal” tolerable (@starbucks @uber @downdogapp @crocs @allbirds @wearfigs @goclove) or diverting resources to manufacturing much needed equipment (@dyson @spacex @nomad and countless distilleries now making hand sanitizer) ⠀⠀⠀⠀⠀⠀⠀⠀⠀
With these efforts, we keep the healthcare system on its feet - a win for ALL ✊🏽🙌🏽

We have all made sacrifices lately, but it is the struggles that make us who we are. Today let us remember the doctors all over the world who have sacrificed their lives in the battle versus COVID19.   • 2 years ago I posted this on @mick_in_japan with the following text ‘Kyoto!! How much I missed you. First thing we see after exiting Haruka Express Train - ART. A gorgeous Mosaic by Hideo Nishiyama created in 1979 for celebrating 100 years of Kyoto Station. And Sakura of course.’ It is still to early too know if I will be able to travel to Japan for my artist residency in September. I am grateful to have many many great memories from Japan to get inspiration from. This challenge is an opportunity for you to share "Why you teach", give a tip, share a video, and/or a resource that can help support communities during this time. 40s yo w B extraarticular dist tib fx on vaca in FL, L open via 14cm semitransverse lac (see pic). “Debrided” & exfixed locally, right casted. Transferred to my hospital for definitive care. .

The fx is quite distal but the lac can be exploited to allow stable, supplemented nailing w almost no additional incisions. .
When nailing ultra-distal fractures find ways to augment your construct. Here: the medial apex of the tibia and the fibula. .
.
SEQUENCE 
1. Supine, diving board 
2. Open lac, D&I
3. clamp tibia via lac
4. nail fibula w 3mm nail **bevel one end of nail to assist w crossing the fx**
5. Suprapatellar approach, semiextended (can do mini-arthrotomy if special equip unavailable!!)
6. Isthmus is tight (chatter at 8.5mm) cancellous bone is dense: **ream meta-epiphysis to minimize fx distraction** path must be perfect! —> load 9mm reamer w ball-tip, un-deployed.  stop reamer at beginning of metaphyseal flare.  deploy ball tip wire 5-10mm, check fluoro, ream, advance wire, check, ream, repeat up to physeal scar.
7. pass nail. distal two interlocks. backslap. 
8. place 2.7mm plate to antiglide via laceration (no new incisions). place apical and proximal screws via lac, no incisions
9. place 3rd interlock ML via lac. place prox static interlock via stab incision. 
10. small rhBMP2 kit to fix site for open tibia
11. close, VAC laceration, splint. .
.
**Total addition incisional burden: - Distal Lat stab for flexinail
- Distal Med stab for dist-most ML interlock
- Distal Ant stab for AP interlock
- Prox Med stab for prox interlock
- 3cm suprapatellar for nail
.
Stable construct w less soft tissue insult than even medial MIPO for this situation.  Take the lemons you’re given & figure out how to make lemonade. .

  • National Doctor’s Day… this chair is not exactly where I thought I would be for this “holiday” this year. Anyone else not feeling very celebratory with what is currently happening on the front lines of this pandemic?

Today I’m grateful there are some things I can do research-wise from home during this period of distancing. I’m worried about my friends and colleagues that are exposing themselves to danger every day as they go in to hospitals across the country, but so so proud of the resilience, self-advocacy, and camaraderie being shown in this incredibly trying time.

The WFH Residency will pay artists who have had arts projects or other work cancelled, or have been made unemployed, as result of the covid-19 / coronavirus pandemic lockdown. It will particularly support projects that take a co-operative or communal approach and connect people while social distancing.

Projects should aim to generate content that can be shared online and/or are physically viewable from wherever you are social distancing.

You can propose activity that is unrelated to art or your practice, and projects unrelated to the covid-19 / coronavirus pandemic are particularly welcome.

Key Dates
Applications are open from 10am Monday 30 March until 10am Monday 1 July 2020. Each month, one submission will be selected for the award.

I started hospital service today. I'll be on for a week. I can't imagine what things will look like in seven days, but for right now its eerily calm. Our ED has been much slower than usual. Our hospital floors are more empty than they typically are. And its a good thing. People are staying home when they can. Elective procedures and surgeries have been postponed. No visitors allowed. Staff that can work from home are at home. .

While much of my day is consumed with re-configuring the schedule at a moment's notice and troubleshooting problems as they arise, today was a good reminder that I am a physician, and that my primary purpose is caring for patients. Today I as given some normalcy in the midst of what is seemingly chaos. Yes, being a hospitalist is busy and hectic. But there is a sense of comfort going from room to room and talking with patients who look to us for their care. For once this week, I knew what my day would look like.

.
 I won't lie. Its been looming in the back of my mind all week--what if I get exposed? After about two patient encounters, that worry was gone. Wearing my mask, and eliminating my white coat, I am as prepared as I can be. This is my job. This is my calling. And I will continue to do my job despite the whisper of fear. .
.
Over the coming weeks, we are expecting the surge. The cases will rise. We will all be called upon to do what we are trained to do. We have to be here, and we will be. So, please, again I ask you to #stayhome and to #flattenthecurve .

  • COVID-19 has sparked a lot of CONTROVERSY.

We have never in a LIFETIME thought this would ever happen to us. Right now the Coronavirus is our present and we hope it will be in the near past. It’s significance on our world is profound, and it’s impact will be written in medical books and history books everywhere. .

But how do we want to be remembered in those history books while this pandemic hit us? How do we want to be described? As a people who distributes essentials to families in need, and the fortunate donating to the unfortunate? OR as a group of people who hoarded toilet paper? As a community that didn’t share face masks they didn’t need at home? As people who couldn’t just stay at home to stop the spread of disease... even if it meant a life? In one of the wealthiest countries, not be able to provide the PPE needed to protect those taking care of the sick? We must not regress as a whole, display racism, selfishness, or animosity.

We have the OPPORTUNITY to write history. Appreciate and follow the lead of the many who are doing good in these times of uncertainty. #Showhumankindness, by:

  • I know I have been pretty absent online lately and I wanted to give you all an explanation/update on my COVID-19🦠 experience. I am sure many of you have experienced the cancellations of events/celebrations that you have been looking forward to, for me it was Eastern States Residency Conference and the Science Symposium at my hospital where I was going to present my research project. As many of you know data collection alone took me 300+ hours and I was so excited when I got a statistically significant result on both my primary outcomes. At first it can be very easy to be bummed out that events we have looked forward to get canceled but after caring for COVID patients in the ICU I am reminded of what truly matters. .

For those who can work from home and practice social distancing please do. For those of you who show up day after day to care for these patients thank you, I know the hardship you have been through first hand. Although I cannot enter the rooms in an effort to preserve PPE, I remain on the floor and round with the ICU team. My preceptor and I sit 10ft away from some of the isolation rooms, we watch all our staff gown up time and time again and take care of these patients. I feel so incredibly honored to be on the front lines contributing in anyway I possibly can but I have been more exhausted these past two weeks than ever before in residency. It’s not just the added work (which I would do over and over again in a heartbeat) it’s the emotional toll that comes with this that is affecting me. For anyone who thinks this pandemic is not a big deal or that they will be fine because they are young, please think again. I have seen patients younger than me put on ventilators and be paralyzed and proned due to severe respiratory failure and I have also seen elderly patients die after contracting COVID from community transmission. As heartbreaking as some days have been, I am so inspired by my coworkers and healthcare system in their response to all of this. I honestly couldn’t be more proud to be on the front lines of this all,

  • I know I have been pretty absent online lately and I wanted to give you all an explanation/update on my COVID-19🦠 experience. I am sure many of you have experienced the cancellations of events/celebrations that you have been looking forward to, for me it was Eastern States Residency Conference and the Science Symposium at my hospital where I was going to present my research project. As many of you know data collection alone took me 300+ hours and I was so excited when I got a statistically significant result on both my primary outcomes. At first it can be very easy to be bummed out that events we have looked forward to get canceled but after caring for COVID patients in the ICU I am reminded of what truly matters. .

    For those who can work from home and practice social distancing please do. For those of you who show up day after day to care for these patients thank you, I know the hardship you have been through first hand. Although I cannot enter the rooms in an effort to preserve PPE, I remain on the floor and round with the ICU team.
