  • Cortical  nephrocalcinosis  is  an  uncommon  imaging f inding.  The  most  common cause  of  cortical  nephrocalcinosis  is  chronic  glomerulonephritis,  in  which  chronic  renal  failure  is  associated with  marked  renal  atrophy  with  smooth  renal  contours. Shell-like  calcifications  develop  in  the  renal  cortex  in  a small  number  of  these  patients  and  persist  even  after successful  renal  transplantation  and  normalization  of renal  function.  These  calcifications  are  likely  to  be  dystrophic  and  related  to  chronic  renal  cortical  ischemia.

  • CHRONIC RENAL INSUFFICIENCY
In chronic renal insufficiency, reduced renal filtration causes phosphate retention and decreased calcitriol synthesis. This results in lowered serum calcium which in turn stimulates parathyroid hormone (PTH) secretion, thus causing secondary hyperparathyroidism. In order to elevate blood calcium levels, PTH increases bone resorption by stimulating osteoclast activity. Osteoblasts respond by depositing osteoid that does not contain hydroxyapatite, which then accumulates along the inferior and superior end plates of vertebra. The aforementioned areas of osteoid appear opaque at radiography due to their increased volume compared to the normal bone, resulting in the appearance known as ''rugger jersey spine'' because of its resemblance to the horizontal stripes on jerseys worn by rugby players

Large, generally homogeneous right renal mass and central low density in mass. 
What is it ? 
Oncocytoma. 
The lesion in this case could represent a solid RCC. A large, lobulated, heterogeneously enhancing mass arising from the proximal stomach. 
Probable Diagnosis : Gastrointestinal Stromal tumor or GIST. 
Although tumor expression of  the KIT receptor is needed for a defi nitive diagnosis of  GIST, imaging, especially CT, has proved to be instrumental. GISTs are usually seen as exophytic, heterogeneously enhancing, rarely calcifi ed masses arising from the GI tract wall. Most of  them, especially larger ones, show heterogeneous density due to the presence of  intratumoral necrosis,  hemorrhagic foci, and cystic degeneration.

The reflections  of  the  parietal  peritoneum  demarcate  the edges  of the  football.Typically,  the  football  sign is  seen  in  neonates  and  infants.  The  most  common  cause  is  spontaneous  or  iatrogenic  gastric perforation,  although  it  may  be  the  consequence of  a  congenital  bowel  obstruction  or  necrotizing enterocolitis.

  • *the dillemma of knee osteoarthritis and Iliotibial band syndrome*

The iliotibial band is a lateral thickening of the Tensor Fascia Lata. It originates from the anterior iliac crest outer lip, anterior border of the Ilium, and the outer surface of anterior superior iliac spine. It encloses the tensor fasciae lata, anchoring this muscle to the iliac crest and receives most of the tendon of gluteus Maximus. It distally inserts on Gerdy's tubercle on the lateral aspect of tibial tubercle. 
In patient's with knee osteoarthritis and specifically in which medial compartment is predominant, it may lead to Varus angulation. This Varus deformity might cause excessive stretching and tightness of Iliotibial band and further causing hypertrophy of Iliotibial band.  Such patient's will present with acute Iliotibial band syndrome and varus medial knee osteoarthritis.

  • *How to use SWI(susceptibility weighted imaging) to your advantage and avoid missing out on small focal thrombosis*

So you're the resident on call at night and at 4 am a patient with stroke comes to MRI. You see multiple embolic foci of diffusion restriction and see the MRA. It's 4 at night, the neurology and emergency medicine residents are surrounding you asking you every minute about the status of vessels, as they say in stroke patient's 'time is brain'. We know, how nerve wrecking this situation can be ! 
So one thing you should do is to always look at the SWI sequence. So this sequence is used majorly to look for hemorrhage or hemorrhagic transformation within the infarct but this can be easily used to look for thrombosis and becomes specially useful in detecting smaller thrombotic focus in branches of major vessels of circle of Willis. 
Now see the images above. There is focal hypointensity which was a thrombus in inferior division of M2 segment of the right MCA. Just on MRA while scrolling it's possible to miss out when you are in residency with a lot going on, during on call.

So the first thing is to identify which image is in phase and which is out of phase. Look at the boundary of the viscera in abdomen, you will see a black 'india ink' outline around the organs which is the hallmark for out of phase imaging. 
Now remember, signal drop of liver in out of phase imaging means hepatic steatosis/fatty liver. While signal drop in, in-phase means iron deposition in liver.

  • FLAIR vascular hyperintensity is a phenomenon on MR imaging that occurs in patients with acute ischemic stroke, intracranial stenoses, Moyamoya disease, and TIA. 
It represents disordered blood flow from collaterals distal to arterial occlusion or stenosis. It indicates the status of leptomeningeal collateral perfusion to vulnerable brain tissue rather than directly visualizing thrombus. Thus, it is not an indicator of infarction but rather of tissue that is at risk for infarction. 
Here this is a FLAIR image of brain of a diabetic patient with large MCA-ACA territory infarct. The red arrows are pointing towards collaterals indicating that there that region of brain was vulnerable for a long my time before it eventually infarcted due to acute occlusion. 
So, while reporting on a brain case of people who are at risk viz old, diabetics, obese patient's, try looking for these collaterals and if you find them, it's always prudent to get an MRA done as a next step. You might saving that from a future catastrophy.

  • *Diffuse Axonal Injury and its underestimation by CT*

Diffuse axonal injury or DAI is one of the most disabling conditions of primary traumatic brain. DAI occurs as a result of acceleration–deceleration/rotational force, and is typically located in the long white matter fibre tracts.
Cortex and white-matter have different densities and therefore rotate at different speeds during closed head injury, leading to misaligned axons or stretched axons (rarely sheared).
The stretching of axons causes depolarization, metabolic alterations, cellular swelling, cytotoxic oedema, and apoptosis. 
CT is often without relevant findings. There might be tiny hyperdense foci (microhaemorrhage) the gray matter–white matter, especially frontotemporal lobes, corpus callosum, especially splenium, brainstem, especially dorsolateral midbrain and upper pons.

In these images we do see extensive brain edema and few punctate hyperdense foci indicating microhemorrhages but grossly they don't do justice  to the patient's extremely poor GCS. 
When we look at the MRI though, we see extensive susceptibility indicative of bleeding in SWI sequences. There is also diffusion restriction in splenium without any hemmorhage which is due to the cytotoxic edema as a result of sheer forces on the axons (qualifying it for a grade 2 DAI) and we also see hemorrhages in the midbrain ( further upstaging it to grade 3 DAI). Also seen is altered signal in the posterior corpus callosum in T2 sagittal image

  • *Diffuse Axonal Injury and its underestimation by CT*

    Diffuse axonal injury or DAI is one of the most disabling conditions of primary traumatic brain. DAI occurs as a result of acceleration–deceleration/rotational force, and is typically located in the long white matter fibre tracts.
    Cortex and white-matter have different densities and therefore rotate at different speeds during closed head injury, leading to misaligned axons or stretched axons (rarely sheared).
    The stretching of axons causes depolarization, metabolic alterations, cellular swelling, cytotoxic oedema, and apoptosis.
    CT is often without relevant findings. There might be tiny hyperdense foci (microhaemorrhage) the gray matter–white matter, especially frontotemporal lobes, corpus callosum, especially splenium, brainstem, especially dorsolateral midbrain and upper pons.

    In these images we do see extensive brain edema and few punctate hyperdense foci indicating microhemorrhages but grossly they don't do justice to the patient's extremely poor GCS.
    When we look at the MRI though, we see extensive susceptibility indicative of bleeding in SWI sequences. There is also diffusion restriction in splenium without any hemmorhage which is due to the cytotoxic edema as a result of sheer forces on the axons (qualifying it for a grade 2 DAI) and we also see hemorrhages in the midbrain ( further upstaging it to grade 3 DAI). Also seen is altered signal in the posterior corpus callosum in T2 sagittal image

  • Whats onodi cell ? And whats the fuss about it ? 
The Onodi cell, or sphenoethmoidal air cell, is a variant posterior ethmoid air cell that extends posteriorly along the superior and lateral aspect of the sphenoid sinus. It is best evaluated in the coronal plane. Onodi cell is an important variant to identify, as the optic nerve commonly courses through the Onodi cell, with a thin margin of bone separating the optic nerve from the underlying air cell. This greatly increases the risk of optic nerve injury during posterior ethmoidectomy, espe-cially when surgeons are unaware that they are in a sphenoethmoidal air cell. An Onodi cell is best visualized on co-ronal sequences by first locating an air cell above the sphenoid sinus and identifying its continuity with a posterior ethmoid air cell.

Pearl: Patients with recurrent pyogenic cholangitis are at increased risk for intrahepatic pyogenic abscesses and cholangiocarcinoma —> it is important to scrutinize the hepatic parenchyma for these entities (neither of which is seen here)

Emphysematous  cholecystitis  must be  diagnosed  early  because  of  a  high  inddence  of perforation  and a  high mortality  rate.

  • Cortical  nephrocalcinosis  is  an  uncommon  imaging f inding.  The  most  common cause  of  cortical  nephrocalcinosis  is  chronic  glomerulonephritis,  in  which  chronic  renal  failure  is  associated with  marked  renal  atrophy  with  smooth  renal  contours. Shell-like  calcifications  develop  in  the  renal  cortex  in  a small  number  of  these  patients  and  persist  even  after successful  renal  transplantation  and  normalization  of renal  function.  These  calcifications  are  likely  to  be  dystrophic  and  related  to  chronic  renal  cortical  ischemia.
