  • Carrying on from my last post, I said I would address each point regarding satiety with its own post so I’ll start with protein and satiety.

Numerous studies have summarised the literature regarding protein and satiety with Paddon-Jones et al 2008 and Westerterp-Plantenga et al 2012 both good examples to read. The general conclusions from these papers are;
Potential beneficial outcomes associated with protein ingestion include the following:
.
1) Increased satiety—protein usually increases satiety to a greater extent than carbohydrate or fat and may enable a reduction in energy consumption under ad libitum dietary conditions.
.
2) Increased thermogenesis—higher-protein diets are associated with increased thermogenesis, which also influences satiety and augments energy expenditure (in the longer term, increased thermogenesis contributes to the relatively low-energy efficiency of protein).
.
3) Conservation or accretion of fat-free mass—in some individuals, a moderately higher protein diet may provide a stimulatory effect on muscle protein anabolism, favouring the retention of lean muscle mass while improving metabolic profile.

I want to look at point one in a little more detail, the study that is often used for this is Weigle et al 2015, subjects in this study were put on a weight-maintaining diet (15% protein, 35% fat, and 50% carbohydrate) for 2 weeks, an isocaloric diet (30% protein, 20% fat, and 50% carbohydrate) for 2 weeks, and an ad libitum diet (30% protein, 20% fat, and 50% carbohydrate) for 12 weeks with each participant acting as their own control.

They found satiety was markedly increased with the isocaloric high-protein diet. Average spontaneous energy intake decreased by ~450 kcal/day, body weight decreased by ~4.9 kg, and fat mass decreased by ~3.7kg with the ad libitum, high-protein diet.

What is also of note in this study is the carbohydrate content of the diet of 50%, they discuss in the paper which has been discussed in the literature that the effect of protein may help to explain the paradoxical weight loss observed in subjects placed on low-carbohydrate diets.

  • What a session! Today’s morning session on “patient safety” was fantastic! Besides 5 great talks, also our position paper on parenteral nutrition was officially launched. Professor Alexandre Lapillonne, one expert involved in the development of the paper highlighted the benefits of parenteral nutrition as a life saving intervention that, if carried out incorrectly, can nevertheless cause harm. Therefore following current guidelines to provide parenteral nutrition safely, is key!

  • Wait a minute before jumping on this. This is the idea is that if weight loss is the aim, people will tend to do better if they’ve bought into a diet concept. This can be something quite general such as low carb or low fat or something on the extreme of the spectrum, such as carnivore or fruitarians. I’ve said this quite a bit but I think diet can be quite religious in that people will swear by an approach even if the evidence doesn’t support is use in every scenario. “But it worked for me”, I’m sorry but the keto and vegan crowd are probably most guilty of this but these are a prime example of a group who have bought into a concept and gone with it.

As a standard, the ideal diet for weight loss should be safe, efficacious, healthy and nutritionally sufficient, culturally acceptable and economically affordable. It also needs to allow long-term compliance and maintenance of weight loss. If we think about this, there is never going to be one diet that ticks all of these boxes for every individual.

The term healthy in that paragraph is also quite an important notion. When looking at the overall scientific evidence about what constitutes a healthy diet it is quite consistent and straightforward: a healthy diet is a varied diet rich in fruits, vegetables, legumes, nuts, seeds, whole-grain products and high-quality proteins and poor in added sugar, refined grains, and highly-processed foods. The amounts of each are very individual and have to take into account the above points plus an individual’s personal goals and preferences.

Long term compliance and maintenance is often the difficulty in any diet with this highlighted in the literature for both low carbohydrate and fat. Behaviour change is very important when it comes to weight loss maintenance with high self‐efficacy for exercise and self‐efficacy for weight management needed. From a dietary perspective James Krieger summarised this well by highlighting that satiety improves adherence with the characteristics of a high satiety diet; • High protein
• High fibre
• Low energy density
• Moderate palatability
• Reduced variety
• Reduced reward

  • It’s key to keep parents informed about the Parenteral Nutrition needs of their little one. Explain to them which nutrients are especially relevant for the baby's development at every stage. Lipids, for instance, play an important role in growth and development. According to the most recent European Parenteral Nutrition guidelines, lipids can be started immediately after birth and no later than on day two of life.

Wie der Namen schon sagt beschäftigt sich ESPEN mit der klinischen Ernährung und dem Stoffwechsel beim Menschen. ESPEN fördert zudem die Forschung 🧪 in diesen Gebieten und bringt regelmäßig neue Richtlinien (Guidelines) heraus.

Zudem findet jedes Jahr ein internationaler Kongress statt, diesmal in Krakau (Polen), vom 31. August bis zum 3. September 2019. Der Kongress steht dieses Jahr unter dem Motto: „Nutrition - A Highway to Health“. Ziel dieses Kongresses ist es beispielsweise die Ernährungstherapie weiterhin zu optimieren und das Wissen in den Bereichen der akuten, chronischen, pädiatrischen und geriatrischen Settings auf den neusten Stand der Forschung zu bringen.

Plötzlich wurde ich aus dem Leben und dem Alltag gerissen. Alle meine Pläne sind zerstört. Jeden Tag bin ich für viele Stunden an Infusionen gebunden. Ich muss immer darauf achten was ich esse.
.
Leider musste ich dieses Jahr, wie auch vor acht Jahren schon, insgesamt zehn Bauchoperationen über mich ergehen lassen und habe dabei sehr viel Darm verloren. Nun muss ich mich mit dem Kurzdarmsyndrom arrangieren. 
Ein Meter Dünndarm von eigentlich 4-5 Metern ist mir erhalten geblieben. Der wichtigste Teil fehlt mir jedoch. Somit werden Nährstoffe nicht mehr ausreichend aufgenommen, ich muss aufpassen was ich esse, das ich alle zwei bis drei Stunden und nicht zu große Portionen zu mir nehme. Zudem darf ich zum, eine halbe Stunde vor und nach dem Essen nichts trinken. Viele gesunde, leckere Sachen darf bzw soll ich nicht essen, wie zB Salat, Spinat, Brokkoli, Äpfel, Kiwis, Beeren etc. Alles Dinge die ich sehr gemocht habe! 
Zusätzlich brauche ich momentan jeden Tag Infusionen (künstliche Ernährung), damit ich keine Mangelerscheinungen bekomme und mein Gewicht halten oder sogar etwas zunehmen kann. 
Das sind nur Ausschnitte der neuen Situation.

Deswegen ist es mir wichtig ein gesellschaftliches Problem anzusprechen, denn ich muss mich nun leider anpassen und diszipliniert sein, doch die meisten Menschen müssen das nicht. 
Es ist erschreckend, wieviele Menschen zu wenig oder eingeschränkt Essen nur um dem gesellschaftlichen Anspruch gerecht zu werden dünn zu sein. 
Jeder ist perfekt so wie er ist!
Ernährt euch ausgewogen, genießt die Vielfalt an Lebensmitteln die wir haben und greift auch mal ohne schlechtes Gewissen zu Naschereien. Ihr lebt nur einmal und das solltet ihr auskosten, wertschätzen und EUREN Bedürfnissen nachgehen, nicht deren anderer.

Somit werden Nährstoffe nicht mehr ausreichend aufgenommen, ich muss aufpassen was ich esse, das ich alle zwei bis drei Stunden und nicht zu große Portionen zu mir nehme. Zudem darf ich zum, eine halbe Stunde vor und nach dem Essen nichts trinken. Viele gesunde, leckere Sachen darf bzw soll ich nicht essen, wie zB Salat, Spinat, Brokkoli, Äpfel, Kiwis, Beeren etc. Alles Dinge die ich sehr gemocht habe!
    Zusätzlich brauche ich momentan jeden Tag Infusionen (künstliche Ernährung), damit ich keine Mangelerscheinungen bekomme und mein Gewicht halten oder sogar etwas zunehmen kann.
    Das sind nur Ausschnitte der neuen Situation.

    Deswegen ist es mir wichtig ein gesellschaftliches Problem anzusprechen, denn ich muss mich nun leider anpassen und diszipliniert sein, doch die meisten Menschen müssen das nicht.
    Es ist erschreckend, wieviele Menschen zu wenig oder eingeschränkt Essen nur um dem gesellschaftlichen Anspruch gerecht zu werden dünn zu sein.
    Jeder ist perfekt so wie er ist!
    Ernährt euch ausgewogen, genießt die Vielfalt an Lebensmitteln die wir haben und greift auch mal ohne schlechtes Gewissen zu Naschereien. Ihr lebt nur einmal und das solltet ihr auskosten, wertschätzen und EUREN Bedürfnissen nachgehen, nicht deren anderer.

  • Started tpn yesterday:( so far so good tho ! It’s been started super slowly because I’ve went so long with so little , my body will be so greatful for this nutrition, the staff on my ward and the intestinal failure team are so lovely and supportive, hopefully weight gain will help me tolerate more feed slowly but surely , ideally tpn wasn’t the right road to go down for anyone but due to my bmi dropping down to a critical stage my team were concearned I was going to go into heart failure if they didn’t intervene so thankfully modern medicine is saving my life yet again 🙏🏻 #piccline #tpn #jtube #parenteralnutrition #gastroparesisawareness

My answers @bravesoul.mel: I have a GJ-Tube, but in the next few weeks I’ll be getting a surgical J-Tube to separate the G and J. I had an NJ for a couple weeks 2 years ago, then got my surgical tube. The biggest benefit of my feeding tube is being able to administer medications, I don’t tolerate formula very well anymore but we’re working on that problem. That being said, this week I learned of a new formula option for me to try out: @katefarms Pediatric Peptide 1.5, it is compatible with my pancreatic enzyme #relizorb (important for Cystic Fibrosis to have pancreatic enzymes). I also learned how to make a drainage bag from a feed bag for times I may run out of Farrell Bags. Lastly, I created a pattern and started sewing drainage bag covers (because nobody wants to see that stuff!🤢).

Dawn was transferred to the Children's Hospital in Newcastle where she was found to have a condition so rare there are less than 100 cases of it described in the world.⠀⠀⠀⠀⠀⠀⠀⠀⠀

Her condition is called total intestinal aganglionosis, which means she has no nerve cells in her intestines, which are required to push food and liquid through the intestines.⠀⠀⠀⠀⠀⠀⠀⠀⠀
⠀⠀⠀⠀⠀⠀⠀⠀⠀
As she is unable to feed normally, she is dependent on parenteral nutrition, an intravenous administration of food that goes directly into the big veins in her chest.⠀⠀⠀⠀⠀⠀⠀⠀⠀

Update time 💖
My life this year has revolves around hospital, I’ve spent more time here than my own home , it’s been 7 months as an inpatient, starting in March , went downhill until the point we were at in August, I say we because at that point , I wasn’t alone , I had everyone on here , flooding me with support and messaged which truly kept me fighting for that little bit longer even when I felt I couldn’t ..... fast forward to today , I finally feel direction in my life , I feel like I’ve definitely turned the corner and I’m miles away from where I hit rock bottom , my body has energy , it’s nourished and thriving from it , for me I finally feel like I have the illness and it doesn’t have me ! 💖 so I have Good news and I mean really good news but I want to keep it a surprise for you guys ! And I’m excited! 😘 also in terms of personal life I’m getting out more , on my day leave I’m going out places and visiting people and doing things that average people do !
