One local business we were honored to support this morning is Elwood Shack, providing breakfast to the ER staff at Regional One in Memphis.   • Cortical  nephrocalcinosis  is  an  uncommon  imaging f inding.  The  most  common cause  of  cortical  nephrocalcinosis  is  chronic  glomerulonephritis,  in  which  chronic  renal  failure  is  associated with  marked  renal  atrophy  with  smooth  renal  contours. Shell-like  calcifications  develop  in  the  renal  cortex  in  a small  number  of  these  patients  and  persist  even  after successful  renal  transplantation  and  normalization  of renal  function.  These  calcifications  are  likely  to  be  dystrophic  and  related  to  chronic  renal  cortical  ischemia.

  • The goal of International Medical Relief dental volunteers on our mission trips is to provide free dental services for patients who have never had or had limited access to dental care. Globally, dental care is limited in developing nations. Providing this much-needed service improves the overall health of the communities IMR serves. Volunteer dentists, hygienists, dental assistants, and dental students, plus highly interested non-dental volunteers all work together in our dental clinics to give our patients the care that they desperately need. We need dental volunteers for every international medical mission trip we take.⁠

  • CHRONIC RENAL INSUFFICIENCY
In chronic renal insufficiency, reduced renal filtration causes phosphate retention and decreased calcitriol synthesis. This results in lowered serum calcium which in turn stimulates parathyroid hormone (PTH) secretion, thus causing secondary hyperparathyroidism. In order to elevate blood calcium levels, PTH increases bone resorption by stimulating osteoclast activity. Osteoblasts respond by depositing osteoid that does not contain hydroxyapatite, which then accumulates along the inferior and superior end plates of vertebra. The aforementioned areas of osteoid appear opaque at radiography due to their increased volume compared to the normal bone, resulting in the appearance known as ''rugger jersey spine'' because of its resemblance to the horizontal stripes on jerseys worn by rugby players

If you are having any trouble getting medications our office task force is monitoring our patients during this critical time to make sure they have uninterrupted testing and medication.

Large, generally homogeneous right renal mass and central low density in mass. 
What is it ? 
Oncocytoma. 
The lesion in this case could represent a solid RCC. A large, lobulated, heterogeneously enhancing mass arising from the proximal stomach. 
Probable Diagnosis : Gastrointestinal Stromal tumor or GIST. 
Although tumor expression of  the KIT receptor is needed for a defi nitive diagnosis of  GIST, imaging, especially CT, has proved to be instrumental. GISTs are usually seen as exophytic, heterogeneously enhancing, rarely calcifi ed masses arising from the GI tract wall. Most of  them, especially larger ones, show heterogeneous density due to the presence of  intratumoral necrosis,  hemorrhagic foci, and cystic degeneration.

A  large  amount  of  free  intraperitoneal  gas  on  a  supine  radiograph  of the  abdomen  is manifested  by  an  oval  radiolucency  in  the  epigastrium,  reminiscent  of an American  football.  The reflections  of  the  parietal  peritoneum  demarcate  the edges  of the  football.Typically,  the  football  sign is  seen  in  neonates  and  infants.  The  most  common  cause  is  spontaneous  or  iatrogenic  gastric perforation,  although  it  may  be  the  consequence of  a  congenital  bowel  obstruction  or  necrotizing enterocolitis.

  • #TBT: This photo was taken five years ago when I was involved in Haiti’s first twin separation. This is a photo of the hard-working nurses who were on call with me that night. They were sleeping on the trays that we usually use to set up surgery. There were no beds available for them to get some rest otherwise. Kind of poignant given what’s going on right now. I was inspired by the photo going around: “not all heroes wear capes.”

  • I usually like to smile in photos, but how can I?
I'm tired... I'm really tired. You can see it on my face. 
How can I smile when Muslims in China are being placed in concentration camps and being ethnically cleansed daily? Over 3 million of them.

How can I smile when citizens in Iraq are being sniped for protesting their basic human rights to live and sustain?

How can I smile when I've assisted over 1200 Afghani refugees off the coasts of Turkey and Greece, fleeing their country and wars pushed by the west?

How can I smile when Kashmir is still under lockdown?

How can I smile when Syrian people are stil being murdered and hospitals are being bombed?

How can I smile when Palestinians are dying and forced out of their homes by a brutal Israeli occuption daily?

How can I smile when children in Yemen are starving to death daily? 
How can I smile when I see what's happening in Egypt?

How can I smile when I see all the mass shootings going on in the US?

How can I smile when black people are still the victims of a poor justice system in the US.

How can I smile when Sudan is still under unrest? How can I smile when many African nations still suffer after many years of colonialism and slavery? 
UNTIL HOW LONG?

I'm sure I forgot many issues....
I ask these questions to bring about awareness. 
I often wonder if this does anything at all? I wonder if telling people about these issues will help them? But then, I remember that when I was feeling down, and my people were hurting, the support and awareness of others really made me feel like I was not alone. So here I am! Supporting and raising awareness for these FORGOTTEN causes. 
So here I am! Here YOU are!

Been MIA for a bit. It's been a long, tiring, yet beautiful time so far here on this surgical mission in Honduras. The Honduran people are some of the most incredible people I've ever met. They come from all walks of life. Generous with the very little they have. Grateful for the little they get. And loving with all their hearts.

  • From the field - My son Matthew and I completed the IMR medical mission trip to Costa Rica over spring break. Matthew’s first duty was to screen and provide reading glasses for over 75 patients so that they could now see.  The smiles and gratitude that he received was emotionally rewarding. Then he was assigned to the triage station where he monitored nurses taking vital signs and documented the patient complaints.  As a medical doctor I felt a profound sense of privilege and honor to be able to care for these underprivileged villagers.   • Reflective moment today✨

I’ve gotten to do some pretty amazing things over the past few days and the great times are nowhere near done. But this morning as I got dressed for my 3rd day of clinic in Yucatán something hit me. I was putting on scrubs that were given to me when I was a 17 year old junior in high school, the very first time that I EVER shadowed a provider! I remember getting to observe ortho surgery seeing things that would probably horrify most, going into rooms and shadowing as patients were being cared for, and feeling absolutely certain that medicine was for me! Now here I am 8 years later putting those scrubs back on to see my own patients, come up with a differential diagnosis, and formulate an assessment and plan in México! Medical missions is something that I’ve always had a heart for and wanted to do and this opportunity is nothing short of amazing. This PA school thing is absolutely brutal but it’s most certainly worth it! 
Moral of the story: work hard and follow your dreams kids, it’ll pay off in the end.

I don’t have kids (moms don’t come for me please).⠀⠀⠀⠀⠀⠀⠀⠀⠀

I met Bilale on the Africa Mercy Ship when he was malnourished and sick, too small to get his cleft lip & palate surgery. From the moment I met him he was ‘Brittany’s Baby’. His mom even told me I could take him as a carry-on when I left. I took Bilale everyday on my shift and strapped him to my back while I passed meds, fed him during the night so his mom could sleep, and loved on him so hard as if he were my own, bc to me all my patients are.⠀⠀⠀⠀⠀⠀⠀⠀⠀
⠀⠀⠀⠀⠀⠀⠀⠀⠀
This is who I am. I don’t share it a lot  because it’s sensitive. I work with the sickest kids. Everyday I want to save my children from the cruel world, the one where there’s lines and tubes and pokes. The world where their sick. The world where they have to meet me. ⠀⠀⠀⠀⠀⠀⠀⠀⠀
⠀⠀⠀⠀⠀⠀⠀⠀⠀
But what I know and believe is I can save them in my time with them. I can be the stand in for my 12h. They will be loved when I am there nurse. I will celebrate their victories, push them when they struggle, make them laugh and smile even on their hardest days. I don’t know what it means to be a mom, yet. But I know what it means to be a nurse. And I’ll keep saving my babies one 12h at a time 👩🏾‍⚕️
